Complete CSCRMQ
To complete the Cyber Supply Chain Risk Management Questionnaire (CSCRMQ):
1. Click the Edit icon next to the name of the form you want to edit in the Form Summary widget to redirect to the form in edit-mode.
2. Read through the information on the Introduction page and click on the navigational arrow buttons to move to the next page.
3. In the Form Questions section, provide answers for all required questions. You must complete all required questions to submit the form.
NOTE: This form consists of a total of 17 questions, each of which includes a text field for additional and optional comments.
4. Select to Save Draft & Exit if you wish to complete the form a later time.
5. Once you complete answering the form questions, and reach the Submission page, enter the Submitter’s Details and click on the Send Response button to submit the form.
6. A confirmation message displays. Click OK to confirm form submission.
NOTES:
- You are redirected back to the Form Details page.
- Your form is immediately made available to your Buyer.
- You do not receive any type of score for this form.
